(High/Low Intensity Rehab & Short Term Complex Medical Management)
Patients on this unit have defined rehabilitation goals and are cognitively and physically able to participate in a time limited, coordinated inter-professional rehabilitation plan of care ranging from low to high intensity through a combined and coordinated use of medical, nursing and allied health skills. Patients may be medically complex with long term illnesses/disabilities. On admission, patients may have limited physical and/or cognitive capacity due to medical complexity, but are believed to have restorative potential.
Within the program, an inter-professional team including medicine, nursing, occupational therapy, physiotherapy, speech language pathology, social work, pharmacy, clinical nutrition, and a stroke clinical nurse specialist (if appropriate) will help to:
• Recover independence in mobility, daily living and communicative abilities/activities through recovery or compensatory/adaptive means
• Work towards individual rehabilitation goals
• Make every effort to ensure the patient returns home or transitions to an alternative living environment
In addition, the Inpatient Rehabilitation Unit houses the Stroke Rehabilitation Unit which is a specialized geographical unit with identified beds for stroke patients to continue their rehabilitation journey.
This program is funded by OHIP. Program eligibility is assessed and determined by a Rehabilitation Charge Nurse based on the ESC LHIN Rehabilitative Care Committee’s Standardized Access and Discharge Criteria. Referrals can be initiated within the hospital’s Meditech order entry system for internal Bluewater Health referrals. For external referrals please call Patient Flow at 519-464-4400 (ext. 5355) between the hours of 8:00 am to 4:00 pm. A physician’s referral is required.
